Sometimes, being put up as the issue owner for a process that went wrong due to a case of multiple defense line system failing felt like the end of days in the company.

There will be countless discussions and reviews that one had to go through even though you have already done your due diligence. And every finger pointing will be directed at you even when it is a collective breakdown.

At least for other whom commit offences might have a chance of wiping their records because of a presidential pardon, but when one caused issues in office and caused some financial losses on something that one cannot control, you will be remembered for life and be quoted as an example. That kind of feeling is really unbearable as people will look at you as some murderer.

And this is why, I hope when issues arise in workplace, we need to look at how the system actually failed to capture a miss out, worked on system fixes to help cover extra ground rather than a sheer human negligence and laziness.
