I got inspired to make this design from others people's entries. I made use of Canva mobile app to do my design and below are the steps taken.

Still with Canva, I was able to generate a QR panel for @hiveph profile (editable to suit the purpose).

And finally, this is the last step which could be edited to suit the right purpose.
