It's like they either didn't see the problem with the ad, or they did see the problem and still approved it because they thought it wouldn't be an issue, or maybe they thought they could get away with doing that.

I think both possibilities are true.

Exactly. Even though good things are happening but if we see the stats, we are still far behind.
