So what's the 2-fa-k problem now?

Lets start with the incident that murdered 180+ innocent people. Mostly elementary age school girls.

Soon after it happen I saw reports trying to link it to a missile from Iran that malfunctioned. Iran denied and that mass media messaging disappeared and questions were asked.

First suspect, a missile from Israel but that was denied. Hmm, that leaves USA.

USA keeps saying it is under investigation and the Secretary of War gets defensive when asked about the killing of children. How can you see such an outcome and defend it?

In the CBC News article...

The researcher says the fact a school was struck shows it was "either the fault of a weapons system or a huge mistake CENTCOM made with intelligence gathering."

So what went wrong?

I accept that it was a precision attack and there was no malfunction with the missile. I am suspicious that the incident is the result of bad AI data in the Tactical Weapon System.

Many of these missile targets come from AI datasets. Now perhaps the filter of the AI was not stringent and it picked based on the name of the building at a point in time that was not current. The target was a school. There is no escaping this fact no matter what the building was before it became a school.
